a wide network of quality physicians, hospitals and other
health care providers. |

Insurance |
The Dental product is offered by Dental Select (www.dentalselect.com) and allows you to visit
any specialist or dentist without a referral. The Plans provide full coverage for preventative and basic
procedures when visiting an in network provider, and partial
coverage for most major procedures. Although you have
the freedom to choose providers, you receive a higher
level of coverage and save significant out-of-pocket dental
costs when you seek care from a Detnal Select
participating provider. |

An employee and spouse may choose coverage
amounts, ranging from $10,000 to $500,000, in $10,000
increments (subject to state restrictions). Eligible dependent
children ages 6 months to 18 years, 23 if a full-time
student, are covered for $5,000 or $10,000 ($100 for ages
15 days to 6 months). |

This employee paid program is administered
by Fort Dearborn Life Insurance Company (www.fdl-life.com)

and pays $35,000 per person to your named beneficiary
if you die from any cause while insured. The Accidental
Death & Dismemberment (AD&D) portion will pay
an insurance benefit of $35,000 if, while insured, you
suffer a bodily injury in an accident and you lose your
life as a direct result of the injury. |

Disability (STD) |
This employee paid program is administered
by Fort Dearborn Life Insurance Company (www.fdl-life.com)
(a wholly owned subsidiary of Health Care Service Corporation,
which also includes Blue Cross and Blue Shield of Texas),
Short-Term Disability protects against the loss of income
for a short-term non-occupational injury or illness. This
company-paid plan Pays up to $500 not to exceed 66 2/3%
of an employee's weekly income. Eligibility begins the
first day of the month following the date of hire for
BOTH Full-Time and Part-Time Employees who qualify. |

| Long-Term
Disability (LTD) |
This employee paid program also administered
by Fort Dearborn Life Insurance Company (www.fdl-life.com)
(a wholly owned subsidiary of Health Care Service Corporation,
which also includes Blue Cross and Blue Shield of Texas),
Long-Term Disability (LTD) protects against the loss of
income should you become disabled. This is an employee-paid
plan, which pays 60% of monthly earnings to a maximum
of $5,000 (exclusive of overtime, bonuses and commissions).
Benefits will be offset by amounts the insured is eligible
to receive from other income sources. Should you enroll
in LTD and become disabled; the elimination period for
collecting benefits is 90 or 180 days. Eligibility begins
the first day of the month following the date of hire
for BOTH Full-Time and Part-Time Employees who qualify. |

Long-Term Disability (LTD) |

An employee may choose weekly benefit amounts from $100
to $1,150 in $50 increments, not to exceed 60% of basic
weekly earnings. The minimum monthly benefit is $100. |

Employees are provided with paid vacation
time. Vacation time is accrued on a bi-weekly basis, and
is pro-rated for part time employees. Full-time regular
employees who work 40 hours a week or more may accrue
as follows:
-> 2 weeks up to 4 completed years of service
-> 3 weeks after 4 completed years of service
-> 4 weeks after 10 years of completed service |

Employees are provided with paid sick time.
Employees may use sick time for personal illness, or illness
of a dependent in accordance with state and federal family
medical leave (FMLA).-> Exempt Employees receive 7
sick days per year with no carryover -> Non-Exempt
Employees accrue 3 days per year with no carryover. |

The Cafeteria Plan allows you to reap cost

and fewer taxes to pay. |
