Benefit Description
Health plans
Medical Insurance Blue Cross and Blue Shield of Texas (www.bcbstx.com) is the provider of American Unit's health care program. (Health Savings Account (HSA) and Preferred Provider Organization (PPO), lets members choose their own providers from a wide network of quality physicians, hospitals and other health care providers.
   
Dental Insurance The Dental product is offered by Dental Select (www.dentalselect.com) and allows you to visit any specialist or dentist without a referral. The Plans provide full coverage for preventative and basic procedures when visiting an in network provider, and partial coverage for most major procedures. Although you have the freedom to choose providers, you receive a higher level of coverage and save significant out-of-pocket dental costs when you seek care from a Detnal Select participating provider.
   
Vision

The Vision product is offered by Dental Select (www.dentalselect.com) and allows you to visit any specialist or care provider without a referral. The Plans provide full coverage for preventative and basic procedures when visiting an in network provider, and partial coverage for most major procedures. Although you have the freedom to choose providers, you receive a higher level of coverage and save significant out-of-pocket dental costs when you seek care from a Detnal Select participating provider.

   
Life insurance
Voluntary Term Life Insurance An employee and spouse may choose coverage amounts, ranging from $10,000 to $500,000, in $10,000 increments (subject to state restrictions). Eligible dependent children ages 6 months to 18 years, 23 if a full-time student, are covered for $5,000 or $10,000 ($100 for ages 15 days to 6 months).
   
Voluntary Life/AD&D Plan This employee paid program is administered by Fort Dearborn Life Insurance Company (www.fdl-life.com) (a wholly owned subsidiary of Health Care Service Corporation, which also includes Blue Cross and Blue Shield of Texas).
   
Life and AD&D Insurance This employee paid program is administered by Fort Dearborn Life Insurance Company (www.fdl-life.com) (a wholly owned subsidiary of Health Care Service Corporation, which also includes Blue Cross and Blue Shield of Texas) and pays $35,000 per person to your named beneficiary if you die from any cause while insured. The Accidental Death & Dismemberment (AD&D) portion will pay an insurance benefit of $35,000 if, while insured, you suffer a bodily injury in an accident and you lose your life as a direct result of the injury.
   
Income protection benefits
Short-Term Disability (STD) This employee paid program is administered by Fort Dearborn Life Insurance Company (www.fdl-life.com) (a wholly owned subsidiary of Health Care Service Corporation, which also includes Blue Cross and Blue Shield of Texas), Short-Term Disability protects against the loss of income for a short-term non-occupational injury or illness. This company-paid plan Pays up to $500 not to exceed 66 2/3% of an employee's weekly income. Eligibility begins the first day of the month following the date of hire for BOTH Full-Time and Part-Time Employees who qualify.
   
Long-Term Disability (LTD) This employee paid program also administered by Fort Dearborn Life Insurance Company (www.fdl-life.com) (a wholly owned subsidiary of Health Care Service Corporation, which also includes Blue Cross and Blue Shield of Texas), Long-Term Disability (LTD) protects against the loss of income should you become disabled. This is an employee-paid plan, which pays 60% of monthly earnings to a maximum of $5,000 (exclusive of overtime, bonuses and commissions). Benefits will be offset by amounts the insured is eligible to receive from other income sources. Should you enroll in LTD and become disabled; the elimination period for collecting benefits is 90 or 180 days. Eligibility begins the first day of the month following the date of hire for BOTH Full-Time and Part-Time Employees who qualify.
   
Voluntary Long-Term Disability (LTD) This employee paid program is administered by Fort Dearborn Life Insurance Company (www.fdl-life.com) (a wholly owned subsidiary of Health Care Service Corporation, which also includes Blue Cross and Blue Shield of Texas). An employee may choose weekly benefit amounts from $100 to $1,150 in $50 increments, not to exceed 60% of basic weekly earnings. The minimum monthly benefit is $100.

Vacation Employees are provided with paid vacation time. Vacation time is accrued on a bi-weekly basis, and is pro-rated for part time employees. Full-time regular employees who work 40 hours a week or more may accrue as follows:
-> 2 weeks up to 4 completed years of service
-> 3 weeks after 4 completed years of service
-> 4 weeks after 10 years of completed service
   
Sick Pay Employees are provided with paid sick time. Employees may use sick time for personal illness, or illness of a dependent in accordance with state and federal family medical leave (FMLA).-> Exempt Employees receive 7 sick days per year with no carryover -> Non-Exempt Employees accrue 3 days per year with no carryover.
   
Retirement and investments
401(k) Retirement Savings Plan It is never too early to start saving for retirement! You may invest 1- 60% of your pre-tax compensation (up to the IRS annual limit). Your contributions are made via pretax payroll deductions.
   
529 College Savings Plan Employees may contribute after-tax deductions to save for the cost of their higher education for themselves and/or friends and relatives for their brighter future plan.
   
Pre-tax spending accounts
Cafeteria Plan The Cafeteria Plan allows you to reap cost saving benefits by paying your medical, vision and dental insurance premiums with pre-tax dollars. What this means is your employee contributions will be deducted from each paycheck before federal and social security taxes are withheld, ultimately resulting in a lower taxable income and fewer taxes to pay.
   
Dependent care (Section 125) Employees are offered to set aside pre-tax dollars for any eligible qualified dependent care expenses related to childcare, and/or care of a disabled spouse or a disabled or aged parent.
   
Transportation (Section 132) Employees can save money by having Transportation pre-tax deductions for:
-> Qualified Parking
-> Air charges
-> Transit Passes and/or Van Pool
   
Work – life balance benefits
Educational Assistance Employees who are interested in continuing their education, may receive reimbursement of tuition and related expenses for job- related courses or degreed programs.
   
Other benefits
Service Recognition Every employee is recognized for their years of service with American Unit, beginning at 3 years and at intervals of 2 years thereafter
   
Credit Union Employees and family members are eligible to join a local credit union.
   
Community Service Day Full-Time Employees are eligible for one Community Service Day (CSD) annually and Part-Time Employees are eligible for one-half day annually.
